Why does clicking the Clear Shader Cache Only button not remove DXCache from AppData\Local\NVIDIA?

Check the logs, if you are not in safe mode, the files may be in-use and so can't all be removed.
So even for this option, it is best to do it from safe mode to be sure no program or the driver itself, are holding the files open.

Only works in safe mode because this path is in use in normal mode.

No it provide every known path including the entire DXCache folder. If it's still there , it's because you tried in normal mode instead of in safe mode and the cache was probably in use (so can't delete it)
